First, the entry may not be completely scored. Be sure that each criterion on the scorecard has a green checkmark to the left of it. 

The gray bubble at the bottom of the scorecard that says "Not all items have been scored" should not be visible.

If you are sure the entry is completely scored, another potential cause is that your browser is not refreshing. This may happen, particularly on Internet Explorer, depending on your specific browser settings. Try refreshing the screen several times to see if it updates.
